Parents are required to answer the following questions in their registration questionnaire in order to complete their registration. Below is the full text of the terms and conditions. Thank you.
COMMITMENT TO PICK UP AND DROP OFF STUDENTS – I agree to be responsible for bringing my son/daughter to class on time (at least 5 minutes before class begins) and to pick him/her up immediately after classes finish for the day. I understand that if my son/daughter is not picked up after 30 minutes and there has been no contact from the parents, Sones de México Ensemble is obligated by the Department of Child and Family Services (DCFS) to call the corresponding authorities.
Do you commit to drop off – pickup your son/daughter in a timely manner?

AUTHORIZATION FOR MEDICAL ATTENTION – In case of a medical emergency where neither of the parents can be contacted, I agree to authorize all medical and surgical treatments, x-rays, labs, anesthesia and other medical and hospital procedures performed or prescribed by an attending doctor or paramedic for my son or daughter, and I hereby renounce my right of consent for the treatment.

RELEASE AND PERMISSION FOR PHOTO OR VIDEO RECORDINGS – With my electronic signature, I authorize Sones de México Ensemble to make any recording of my image, voice or music and that of my son/daughter, including, but not limited to photography video, audio or interviews on any electronic or digital format. I understand that the recordings may be used for educational and promotional purposes including but not limited to promotional publications, brochures, posters, catalogs, manuals, workshop materials, billboards, websites, news, broadcast, print media, or any type of publicity. In addition, I agree and understand that there will be no financial compensation or special considerations for me or my son/daughter or our heirs, agents or other assigned beneficiaries at any moment due to my participation and/or that of my son/daughter in the above mentioned activities for the use of my voice, my image or that of my son/daughter.

Es requerido que los padres respondan a los siguientes términos en su formulario de registro para poder finalizar su inscripción. A continuación están los términos y condiciones completos. Gracias.
COMPROMISO DE RECOGER A LOS ESTUDIANTES – Soy responsable de traer a mi hijo/a a clase a tiempo (por lo menos 5 minutos antes de comenzar la clase) y de recogerlo/a inmediatamente al terminar la clase. Yo entiendo que si mi hijo/a no es recogido/a después de 30 minutos y no ha habido comunicación con los padres, Sones de México Ensemble está obligado por el Departamento de Servicios para Niños y Familias (DCFS) a llamar a las autoridades correspondientes.
¿Se compromete a dejar y recoger a su hijo/a a tiempo?

AUTORIZACIÓN PARA ATENCIÓN MÉDICA – Autorizo todos los tratamientos médicos y quirúrgicos, rayos X, laboratorio, anestesia y otros procedimientos médicos y/o hospitalarios que sean realizados o recetados por el médico tratante y/o paramédicos para mi hijo/a y renuncio a mi derecho de dar consentimiento para el tratamiento. Esta renuncia sólo se aplica en un caso de emergencia donde ninguno de los padres o tutores puedan ser localizados.

PERMISO DE FOTOGRAFIA O VIDEO – Al firmar aquí autorizo a Sones de México Ensemble para hacer cualquier grabación de mi persona y/o la voz, música, o imagen de mi hijo/a, incluyendo pero no limitado a fotografías, video, audio o entrevistas en cualquier medio electrónico o digital. Entiendo que las grabaciones pueden ser utilizadas con fines de relación educativa y/o públicas, incluyendo pero no limitado a las publicaciones publicitarias tales como folletos, carteles, catálogos, manuales, materiales de talleres, pancartas, sitios de Internet, noticias de difusión o de impresión de anuncios o cualquier tipo de publicidad. Además se entiende y estoy de acuerdo en que no habrá dinero o consideraciones especiales para mi, mi hijo/a, nuestros herederos, agentes u otros personas asignadas, en ningún momento debido a mi participación y/o la de mi hijo/a en alguna de las actividades mencionadas aquí por el uso de la voz o imagen mía y/o la de mi hijo/a.
